Personal Information Request Form

This form will enable you to request access to personal information held about you by the federal government. With this form, you may request copies of your previously submitted Social Insurance Number applications.

International SIN Application form (International Applications only)

This form can be printed for completion.

You must submit the completed application form to Service Canada by mail. Remember that it is necessary to provide original proof-of-identity document(s) and the fee for a replacement card (if applicable). For details of how to apply for a SIN, refer to "How do I apply".
