What forms do I need to apply?
To apply, you must complete, print, sign and return our Application for the Old Age Security Pension (Form Number ISP3000), which can be found on the Forms Profile page.
Important: The following information will assist you before you begin completing the application for the Old Age Security pension.
You should apply for the Old Age Security Pension (OAS) six months before you turn 65. Normally, you must apply on your own behalf. If you are applying for someone else, please contact us for more information.
The Forms Profile page for the OAS application provides basic information about the form and how to complete it. There are also links to additional information:
- Old Age Security Program – where you will find additional information on OAS payments, benefits, taxation and more.
- Returning the Form – where you will find a list our Regional Office addresses. Please mail your application form to the nearest office.
Both the HTML and Portable Document Format (PDF) versions can be filled online. You will need Adobe Reader to complete and/or print them. A free download of Adobe Reader is available online. After installation, you may have to close your browser or reboot your computer for Adobe Reader to work.

The form, which is part of the Old Age Security pension application kit, consists of two parts:
- The first part, pages 1 to 4, is the actual form that must be completed, signed and returned to us.
- The second part, pages 5 to 8, contains detailed instructions to help you apply for the OAS pension. You do not need to mail these pages with your application.
You may also contact us to request that an application kit be sent to you by mail, or you may pick one up at a Service Canada Office.
You should mail your completed and signed form to the nearest Service Canada regional office. If you are applying from outside Canada, mail your form to the Service Canada regional office in the province where you last resided. See the list of our regional offices.
