How do I apply for the Allowance?

To apply for the Allowance program you should:

• complete an application form from our Web site, print it, sign it and return it to us; or
• call one of our Service Delivery Agents at 1‑800‑277‑9914 (TTY 1‑800‑255‑4786)and we will send you an application kit.

Please make sure that you complete your application form and return it quickly. If you do not apply right away, you might lose some benefits.
