Any future Canada Pension Plan benefits you receive will be based on your contributions, earnings and personal information appearing on your Statement of Contributions. Check that this information matches your income tax slips and returns. If any information is incorrect or missing, please follow these instructions:
To correct your date of birth
The date of birth on your Statement is the date of birth on your Social Insurance Number record. If your date of birth is registered incorrectly, we suggest you call the Social Insurance Registration office at 1 800 206-7218 (option 3). If this is incorrect, you will need to complete an application form and provide your original provincial birth certificate, Certificate of Canadian Citizenship or immigration documentation. To correct your contributions
If you were employed, send a copy of the earnings and contributions information on your T4 slip(s), or a signed letter on company letterhead from each of your employers confirming all your earnings and contributions to the Canada Pension Plan for the year(s) in question.
If you were self-employed, send a copy of your T1 annual income tax return with your earnings and contributions information and your Notice of Assessment for the year(s) in question.

To inform us that the person indicated on this Statement is deceased
Send a copy of the death certificate or funeral director's Statement of Death and the SIN card to the Social Insurance Registration address below. If you do not have the SIN card, send the top portion of this Statement of Contributions.

You can request that an official Statement be mailed to you and you can view and print an official copy of the most up-to-date version of your CPP SOC at any time by visiting the Statement of Contribution online service.
First time users must have a Personal Access Code. If you don't have a Personal Access Code, you can apply for one online. Once you have your Personal Access Code you have to register for My Service Canada Account. You will need to create a User ID and Password during the registration process – this is called an epass. If you already have an epass, you will not need to get a new one. You have to enter your information on the epass login page. On return visits, you will only need to enter your epass user ID and password to access My Service Canada Account.
