What forms do I need to apply for the Common Experience Payment?
You must complete a Common Experience Payment (CEP) application form to provide the information we need to process your application. The settlement agreement states that no eligible CEP recipient will receive payment without submitting a CEP application.
Please provide all information requested on the application form, including all required signatures. Incomplete applications will result in processing delays.
