What should I do if the name I use on my application is different from the name on my identity documents?
If the name on your Common Experience Payment application is different from the name on your identity documents, you must provide the original or a certified copy of one of the following documents:
- marriage certificate or registration;
- divorce decree;
- legal change-of-name document; or
- adoption papers.
If you cannot provide an original or certified true copy of one of these documents to verify your name change, you may submit a Guarantor Declaration.
