NOTE: Help on accessing alternative formats, such as Portable Document Format (PDF) files, can be obtained in the alternate format help section.

If you are not satisfied with the response you receive to your initial application, you may submit a request to have it reviewed by filling out the Request for Reconsideration form. (PDF version, '29.3' kb)

If you are not satisfied with the response you receive, the settlement agreement gives you the right to appeal the decision to the National Administration Committee.

You will have received a letter that provides details about the decision. The letter you received outlines the steps you can take to appeal the decision. Included with that letter is an appeal application form.
