Please note that not all provinces offer fax or in person orders. Please consult with the list below to find out how to order ROE forms in your province.

Non-Canadian businesses operating in Canada

If you are a non-Canadian employer who wants to order ROE forms, your place of business registered and operating within Canada must follow the same procedures as Canadian businesses.

What we will ask when you order ROE forms

The person taking your order will ask for:

  • your business number - employer's number- issued by the Canada Revenue Agency (CRA),
  • the name, telephone number and address of your business.

If you do not have such a number, you will have to request it from CRAGovernment of Canada site before you place an order.  

If you plan on ordering in advance and picking up your ROE forms in person, you will  be asked to provide two pieces of identification, such as valid driver's license, photo identification card, major bank/trust company credit or banking cards.
